Sway bar D link popped out

I didn’t fix the annoying noise but it was probably a good idea to get new ones anyway as the old ones were fairly worn.
Driving into the driveway at work today (slowly and on an angle as not to scrape), I heard a loud pop/quiet bang which make the original creaking more prominent. Parked the car and had a look. The D bush had popped out from the bracket and was hanging loose around the swaybar!?
Anyone know what this is caused by? Under-tightening? Over-tightening? Under-greasing? Over-greasing? There's nothing I can do about it until I get home tonight so I'm keen to get some thoughts before I have the same thing happen again tomorrow..!
I used Castrol LMM grease.

How hard do u have the sway bar setup?
Currently on the hardest setting.
I originally installed the RSB in Jan on the medium setting. That's when everything started creaking. (HD mounts, HD links, 22mm RSB and lateral link strengtheners at the same time)
Put it on the hardest setting about 6 months ago.
New D bushes last night.

Rubber ones are quieter but don't last as long.
Still, after replacing the worn ones, the creaking is still evident. Unless something is wrong with the mounts, my guess is the struts...

Mine has a groove in which the bush compresses into on the D.. but mine isn't Whiteline (Cusco or something) that seems to stop it popping out..
did you grease the outside of the bush? i don't think you should have.. only the inside needs grease.. and even then, only a light spray with something like Lithium grease is all i did..

I did grease the outside of the bush but only because I've read it's best to 'grease anything that touches metal'.
I've also read to 'pack it with as much grease as possible'!
I suppose you just have to find what works best for you..?
Maybe a little less grease?

I use to use spray on lithium grease but found it didnt stick well to the bushes so I started using molybdenum grease...
The LMM has moly in it and was recommended by most on Ozfoz. Seems to be working well... I think the creaking goes beyond the bushes.
Cam, anything I can do to the mounts to quieter them down. While I was under there yesterday I tightened them up... But I'm not sure where to go from here in that respect.

Springs can creak in the strutts and not be related to the strutt itself, shitty rubber or fitment in there. In our old 4x4 the creaking was almost always a dead steering ball joint. So it can be anything.
