It doesn't matter. It goes on your taxable income at the end of the Financial year. In whatever tier you are because of your taxable income for the year, you will be charged that MLS, unless you have hospital cover, in which case, you don't pay the MLS at all, regardless of whatever tier you are in

 

The variables that determine how much you will pay for your private health care will be your age, marital status and State. They don't care how much you earn. Lol!
